Can someone please let me know what exactly are the looking for when this is listed with a job posting
'please include SALARY HISTORY with your resume"

Where exactly do I list this?
In the cover letter? ( how) On my resume? (IF SO WHERE)

Any advice is greatly appreciated

Your salary history should be included as a separate page in your papers that you send, not as a part of your cover letter. It is best to leave it separate from your resume too. Do the same if you need to include a list of references. Put your items in this order when sending:
Your cover letter
resume
salary history
any references or other documents they requested in the job ad
